Feature Request: Add Description to Custom Reports in Forms

Over time there are quite a few reports that can get created within Forms. It would be nice if there was a description field where we could include a description of what the report is for to make it easier for users and administrators to maintain their systems. This can help with not creating additional reports instead of using a report that might already exist, or knowing why someone created a custom report to begin with and decide if it is still needed.
