I'm working on a Form that would allow our Legal team to add bulk holds to records. This would be mostly based on the type of document needing the hold within a certain account number range. 

The issue I'm having is returning a list of available document types. The document types are an index list value on each template. The attached screenshots show my Forms query to pull back all available templates. I then need to allow them to select the document type from that template they wish to apply the hold to. I was hoping to find this in the LF database somewhere but am coming up empty. 

I'd like to do this part with just a Forms direct query, but can use Workflow if that's the only way.
