Event Registration and Administration Missing Info

I am back trying to get this process to work.  I have been able to download the forms and get them working for the most part but, there seems to be some data missing.  
 

On the Event Registration RSVP Form there are a few CSS classes used on the form, but the code that goes with them is no place to be found.  

Can anybody help with this?

Hi Jason,

I worked on this template so I can speak to this, thanks for your patience!

The CSS classes were more for the division of components of the form. The CSS does not interfere with the forms functionality, and the template did not have any actual code attached to it.

For your implementation, you could use the provided CSS classes to be able to apply your own formatting either by each individual section or all sections based on the classes used.
