Form Matrix for Performance Review

I am trying to migrate a word document into Forms.  The issue I am having is that I am trying to create the matrix where the checkboxes match with the ratings.  There are 12 factors in total.  Is there a way to create in forms the same as the attachment?  I am on base version 11 and am trying to use the new layout designer.

This isn't the most popular opinion, but I would not try to make Forms look like the Word document. Use Forms for how it was meant to be used. Instead of having a different column for each rating, create a radio button with the different rating options or a drop-down field. This will make reporting a lot easier. I did this at a school district I worked at for their employee evaluations and it worked like a charm.

We haven't done this in the Modern Designer, but we do have our Performance Evaluation formatted with a likert scale in the Classic Designer. 

 

We got the CSS for this formatting from a process named "CSS/JavaScript/Formula Examples" in the Solutions Marketplace (back when it was called the Business Process Library).
