I need help with this Activity:  Set Records Management Properties - Set Event

I created a Record Event of Employee Terminated and tried to get workflow to set this event on folders that are newly terminated employee folders based on a few triggers, but in testing, the result is not quite right.

And this is what I end up with.  Note that it somehow reflects an Approved for Destruction event that I did not ask for and the event date is not set. And the folder is not cutoff.
