What would cause Forms to start saving the resulting records with these field types showing? It's messing up our forms terribly! And it is not on all submissions, just randomly two form record results amongst many!

These were added to meet WCAG standards, however normally they should be hidden.
Could you check if there's customization on forms that may break this? CSS class or Custom JavaScript?
They are mainly under the CSS class "screen-reader-legend".
Thanks for your response, Yuxuan Wang. I don't have that kind of CSS or JavaScript on any of my forms.
Co-incidentally, I was talking to our LF support team about this today in a scheduled meeting about something else and in the end we agreed to call this some kind of weird glitch. It happened twice this week, two different forms, forms that get multiple submissions each day yet only one glitchy result for both. AND... in each case, when we restarted the steps prior and moved forward again, the end result was no glitches, just the expected proper records.
We are going to leave it at that... hoping that it was just some internet connection bump or something and hoping it doesn't happen again.
