Recently, I came across a few POs review process has been completed, but not send to Laserfiche (please see image attached)
Why does this keep happening? and how do we resolve it?
Has anyone seen this happen before?

You would have to look at the step in Forms admin and see what it is stuck on. Since it works mostly and only fails on some of them there most likely is some missing info or some "bad" info that has prevented the save step from completing. You would have to dig into the failing one and see what Forms says the problem is. You should be able to go to the step that is in progress and get details on the problem. I've had this happen before and have been able to find out what is holding it up and get it fixed.
Ensure the User used in the Save to Profile has access/rights to the repository where you are attempting to save the documents, as well as Metadata Templates and Fields, and that there are no data mismatch or field constraints that would stop the fields from being saved. If using Forms OnPrem, check the Event Viewer on the forms Server for Forms errors.
