"Laserfiche Forms has encountered a problem" during the Save to Repository step

I have already put a ticket in with LFC Support but I just wanted to throw this out here on the off chance somebody has seen something like it before.

One of our customers periodically sees forms saved to the repository as an error message rather than the actual form. It looks like this:

That's the 'form' as saved to the repository. No errors are generated in the event logs when this happens and the business process runs to completion. All the metadata associated with the form is there. There's just no form, only an error message.

One thing I notice is that the Save to Repository step takes about two minutes where usually it's is just a matter of seconds. It's intermittent and fairly rare but it happens often enough that they notice it. It's not an issue with any particular business process. We've seen it happen in several different ones over a period of time. Has anyone else ever seen anything like this?

We are on Forms 11.0.2201.20436.

We are being told this is a known issue and are being advised to update to Laserfiche Forms 11 Update 3 or later.
