Workflow Service Task Not Proceeding After WF Completed

I have a form where a Workflow Service Task is not proceeding after the workflow is completed.  The WST says it is processing but wont complete.  This has happened a couple of different occasions.  We are using LF 11.

If you look in the Failed Instance Completion node in the Workflow Admin Console, is the workflow instance listed there? If it is, it should have the error it ran into trying to notify Forms. If it's not, then as far as Workflow knows, Forms got the notification.

There is nothing listed in the Failed Instance Completion node in the Workflow Admin Console.  What would cause Forms to not get the notification?
