There are no built-in calendar field types within Forms. Rather than try to recreate what is already available using Outlook, you could use the Workflow activities to create Outlook calendar items. You could still use Forms if you wanted to for people to book appointments and then kick off workflow to create the appointment on the corresponding Outlook calendar. If you are using O365 though, there is an O365 app called Bookings that allows for this as well.
