Is there a way to route a specific form to be an attachment later on in the process?

I have a form that is filled out by an anonymous user and when it arrives to the office, the office user attaches a copy of the student's Birth Certificate, Immunization Records.  The Immunization Record gets sent over to the nurse for their review and the nurse uses a different form than the original registration form.  When the nurse approves or rejects the Immunization Records I would like to make their form an attachment that can later on at the end of the process can be added along with the copy of the Birth Certificate, Immunization Record and the original Registration Form to the Repository.
