Retail Food Establishment Inspection Report

Greetings All,

We are exploring moving our Retail Food Establishment Inspection Report (aka health inspection) to a Laserfiche form. 

Not having anyone well versed in creating forms in Laserfiche, this form looks daunting and overwhelming - see the attachment for the form currently in use.

The overall look of the form seems complex and having to capture Points for items that are not in compliance, sum them and perform a calculation to determine the Total Score seems to add another layer of complexity.

Does anyone have suggestions how to build the form?

FYI...Our forms version is Professional Version 11.0.2201.20436

You're in luck that v11 handles side-by-side fields really well (without the need to write/understand CSS code).

I've attached a mockup of how I would probably approach this form build.

This is just a number field for the OUT field, checkboxes for the other options, and HTML for the question.

The total field is a simple formula, and I've attached a screenshot of that as well.

I would agree with @████████- sticking to the new designer for a complex structured form like this is going to be a good bet.

You might want to consider upgrading to Forms 11 Update 3 (you are on Update 2) - it added a lot of helpful functionality to the new designer, including a lot of great improvements to Field Rules that could be very helpful for a process like this.
