Placing an annotation on a document requires that the user have an exclusive lock on the document. This happens automatically, but prevents users from placing annotations at the same time.

Once a user acquires a lock on a document, other users will be unable to modify the document until the first user saves their changes.
I would take a step back and look at your business processes. Why are multiple users attempting to annotate the same document at the same time? Are they duplicating effort? Could a workflow route it to them in turn?
