Forms does use both JQuery and Bootstrap, but I don't think it uses the specific versions you indicated.
I can't test 10.4, but in the case of Forms 11 Update 3 - I went digging around in the folders on my test server and I'm seeing items labeled as follows:

Many of those folders have text files in them instructing not to upgrade them due to the fact that they have been modified for use by Forms.
