Multiple questions in a second column

Hello Everyone,

I'm trying to duplicate something like this (mocked version of actual form).

I've got the first column (one question of checkboxes and added 'Other' fields) showing as I want. It's the second column with multiple questions and multiple types of data that I can't seem to work through. Inline-block seems to work only on one line, unless I'm doing something wrong.

I tried the New Designer (V11), but again, I could only get one question to show in the second column.

Any thoughts??

You can add a checkbox input with only 1 selection, there is no need to share all selections in the same object. This way after they check, say Abdomen Press, a field rule will show the details they need to enter underneith that checkbox.

Just don't use the field title when breaking your fields up to multiple objects, create your own title or use the first title only.
