Radio button choice not reflected upon loading of first user task after original submission.

I have a form that will not show the radio button choice at the approval stage (the first user task).  I have tried removing ALL javascript, CSS, field rules, etc and cannot figure it out.  I have checked that the variables after the original submit does reflect the choice the person made on that radio button.

Weirdly enough, the next user tasks reflect the radio button choice, but if I copy the last form and use it exactly as is in the first user task stage, it does not! 

Can anyone point me to something else that might be causing this form to not show the choice at that stage?

Since you said there is JavaScript so I assuming you are using the classic form designer not the new form designer right?

Can you take screenshots to illustrate the choices&values configured for the radio button? Please also take a screenshot for the radio button value from the instances details->variables list.

Thanks, Xiuhong.  There is absolutely no JavaScript related to this field.  I just removed everything in my attempt to see how far back I had to go to find what was causing the problem, but it didn't matter how much of anything I took out, it didn't change the results.

There are field rules where different other fields pop up when one of these choices are made.  I removed those too, but it did not fix the problem.

Actually, I think I may have found the problem today.  I decided to change the wording in the choices to something more basic to see if it was something in the wording it did not like, but while setting that up, I noticed that the values for those choices was different on the application and on the approval form.  I ran a test after making everything the same and it worked!  Doing some more testing now with putting the long sentences back in.
