Forms 10.4.5 Upgrade Database connection problem

I'm in the process of upgrading Forms from 10.4.2 to 10.4.5.  The upgrade looked like it was going okay.  It seemed to update the version of forms and asked to update the database.  I said "ok" but now when I go to forms configuration at the Database settings, it's asking to "Select the database server that Forms will connect to. Saving changes to this page will restart the Laserfiche Forms Routing Service."  The correct SQL server and database name are listed.  When I try to login I just get an "Unexpected Error Occurred" message.

 

I was wondering if the database is still in the process of updating in the background but have no idea to know how one would tell if it's still going on.  Any ideas of other troubleshooting steps?

 

In the Forms Admin event log, I see:

If the update is still in progress, when you open the login page, you will be redirect to the error page with  message says "The Forms database is from a previous version and must be updated." instead of login page. 

Can you please paste the full error message? We can tell where the error is come from with the full stack trace?
