Quick fields is not assigning captured data

I have looked at this so much I am crazy.  We have a batch document session that is separating the the documents for me but it is not assigning the date to my new document.    I do have the document content set and it is not working.  I had 18 pages and after it separated only 4 of the documents had dates.

This session has been working for months without an issue and now it is acting up.  Not sure what else too do.  I have repaired the software and even went so far as to rebuild the session and I still get the same problem as shown below.  All I am trying to do is assign the document date.   

 

Does anyone have a suggestion on what I can try?

That does sound frustrating, I'm happy to help take a look. Can you post a screenshot of the metadata configuration showing where you've added the date token?

sorry.  We had a server go down last week and this is the first time I have had a chance to get back to this.  It will not let me add the date token any more from the information retrieved.  When I try to apply the token it not longer show up as a choice.  We just upgraded to 11.0.0498.
