Is there a way to put a table within a collection. I have tried and cant. 

I am wanting to create a form where the submitter would be selecting multiple accounts and for reach account would be able to select multiple people and select their approval rights. 

So I was wanting to have a collection that had Department and department code then a table within where they could add multiple people as an approver and select the rights. 

Is there another way to do this. I am trying to avoid people have to submit multiple forms and/or having them have to fill out the department multiple times on a table.
