It's not very clear, but I think you're referring to the image pages and the electronic document component of an Laserfiche document? If you generate image pages for a Word document, they are not kept in sync when the Word document changes. So it is possible that your Word document's content was modified significantly after image generation. Or you could have some other background processes that appended these pages (correctly or not).

Audit Trail may tell you who made which changes and help you track down what happened.
