If your goal is to save all approved forms to one folder and all rejected forms to another, you can do all of that in Forms without needing a workflow.

After the approval task, use a gateway to make two branches, one for approved, the other for reject. On each branch, you can put a save to repository task with a different folder and different metadata configured.

Don't use workflow for this case, it's not needed. If you wanted to call a workflow, you would use the Workflow service task within Forms. That will kick off the workflow, then you use Retrieve Business Process variables on the Workflow side to grab data.

This short video covers exactly how to set up the Forms to Workflow connection. It hasn't changed between 10.3 and 10.4. On the Forms side, just use a workflow service task to call a specific workflow. On the workflow side, use a retrieve business process variables task to select which variable values you want to pull into the workflow.

I'm guessing you want Workflow to pick up the entries created by Forms and move them to a different folder?

You could give your workflow a starting rule based on entry creation in whatever folder Forms uses. In that case, you wouldn't need Find Entries since Workflow would have a starting entry to work with.
