How to move the title from the table field to the side like a single line??

I have Current Name on top of the text box field in the Table Fields how can I get it to the side without using the Row Field in the Table Field and get it like the example picture??

Try using a collection. It will allow the user to add rows (just like the table), but it will be in the format you want.

Here my next question how to hide just the Field name and text below after you click on the Add button I just want to see the Field name and text below one time??
