I'm trying to format a small table in Forms to be viewed in Row format.  I am able to move the input field into Rows using CSS, but not the column headings.  Does anyone have some sample code that makes this possible?

Ideally I'd like to place the Column header to the left of the input field.

Any help is much appreciated.
