Am I understanding correctly that the form shows the usual Submit/Approve/Reject buttons, but clicking the buttons has no effect?

If so, then it sounds likely to be something related to the form validation - such as a required field that is missing - if you don't see any fields showing validation errors, is it possible that there is a hidden field with errors?

Also - it would be a good idea to edit your post and change it to a question instead of a discussion - that way you can mark it as resolved once complete.
