Include comment in action history details on Laserfiche Forms

Hello,

Can you please advise how to go about adding the comments under the action history as displayed in the screen shot above? LF Forms Version 10.4.0

Those comments are made by the approver/reviewer assigned to the user task. At the bottom of the user task form, the assigned user can type in comments that are added in action history for auditing purposes.
