I have never used a list import before so I am trying something Basic:

Create a folder and name it

Place document from monitored folder  into created folder 

assign template & populate fields.

The result I am getting are as follows:

Creates folder and names it, places an empty document in the folder and names it using the ss number, does not assign the template then dumps the 4 images into the root of the destination folder assigns the template with none of the fields populated;

Here is the list Import I an using:

JOHN

ENDFIELDS
DOCUMENT(111-11-1111)
STARTLIST
