You should be able to use a <video> tag to embed your video using a custom html field. (see documentation here) Alternatively, if your video is coming from a platform such as YouTube or Vimeo they should have an embed option which provides the html tag needed and you would just copy paste into Forms. You can usually find this under their share button. Hope that helps.
