What does it mean when Quick Fields creates a document called Needs Attention - QF? I see nothing in the Quick Fields session that would name it that.

Quick Fields only creates the documents that were in the session. Is it possible that you have a workflow doing further processing on documents as Quick Fields sends the repository?
You may be able to tell from the document properties (Created Date, Last Modified Date, Modified By) or, if you have Audit Trail configured, get a full report of all events on that document.
Yes, there is a workflow that kicks off and will send items to Data Correction folder. However, the workflow renames it to a completely different name. Maybe there is another workflow I am unaware of then?
What is the Audit Trail? Do you mean searching for the Workflow name within Workflow and looking at the instances?
You can search in the Workflow Designer by entry ID and see everything that ran on the document.
Laserfiche Audit Trail is a feature of the Laserfiche Server where all user activity on documents is logged and available for searching for privileged users. It comes included with Rio, but it does need to be turned on. You LF system administrator should know more about whether it's used or not.
