Creating a form in Laserfiche to mimic a mandated form

I have to create a form that looks exactly like a mandated form for one of my customers. Is there a way to embed the image of the form and create fill in fields on top of the image? Would I have to use CSS to do so? I know so little CSS that it would be difficult to do.

It would require CSS, to set the background and position of the fields.

I wonder why the form needs to include the formatting of the original form, why not just the same fields and instructions.

If you only need it to look like the original form in it's final archive, and not when being filled out, check out the Fill PDF Fields activity in workflow, which allows you to fill a fillable PDF with the data submitted.
