Workflow - How is it possible to skip an activity without a conditional sequence

I don't see why my activity is being skipped when there is no conditional sequence around it.

If you published an update to the workflow definition and updated running instances, this is expected. If the instance was already past that point when it got updated, then the updated definition displays it, but there is no record of it running in this instance.

This is exactly what happened. It was added to the workflow at a later date, and it shows up in the history. I did not realize that the latest workflow definition would be overlayed on the history, seems that sometimes it wouldn't even be logical, like if you deleted an activity for example it would look like an action happened without any activity to show for it.

Workflow version?

Did you confirm the activity actually was skipped as evidenced by the task not being created in Forms? Would want to rule out a UI-only issue first.
