Forms Signature field and date field not displayed in the correct format

I have a scenario where a laserfcihe form is submitted and it goes to an employee for approval. There is an acknowledgement blurb which the employee reads and signs in the approval window. And Acknowledges the form. When the form gets submitted a copy of the document gets filed in the repository and a copy is emailed back to the employee. 

The employee sig and date columns appear a lil different in the pdf. It gets moved to the next line. How can i fix the display issue?

I tried removed the blurb and made the column size from medium to large. Nothing is helping.

The "Employee Signature" field doesn't look like an image, is actually a single line field? Does the form look like the same if you open the employee submitted form from the Monitor page? Do you have any customize CSS on the form? Can you copy the process and remove the custom CSS and check again?

Yes, it's a single line field. I checked the monitor page and the formatting of the signature and date is alligned correctly.

I don't have major changes on css on the form. The only change i made is to hide the employee signature and date fields  display hidden on the form when the manager initiates the form. And these fields show up when the employee receives the form to acknowledge and not when the manager initiates it.

I played around with the hidden fields and that is what is causing the misallignment.

So if i make the signature field and date field to show when the manager initiates the form the allignment is correct. Not sure why it does not like the fields to be hidden .

I created a similar form with last four fields as your screenshot and can't reproduce it.

If you would like to investigate why it doesn't work when the fields were hidden, can you please open a support case with the download process that can reproduce it?

IYes i can open a support case. This is the second project it's happening to me.  What is the procedure to start a support case? Thanks again!
