We have a spontaneous issue where the approve/reject buttons do not appear on the form after the user selects assign to me. However, it will then appear on the completed tasks and the form shows as approved in the monitor tab. How is this even possible since the approve button never showed up so it was impossible to click approve? Keep in mind, this exact same form works fine 99% of the time. This just happens randomly every few forms.

Please open a support case through your VAR so we can investigate this process more closely. Is it always approved by the person who clicked assign to me? Is it possible another teammate grabbed and submitted the form before that person could claim it?

Yes, it is always approved by the person who clicked assign to me. It is not being grabbed by another user. In the monitor tab, we can also confirm that the right user shows that they assigned it to them. Are there additional logs we can look at?
