What is the best way to create a Likert Scale in Forms like the below image?

If you go to the business process library, download a process called "CSS/JavaScript/Formula Examples". There is an example of exactly this on the third page called "Radio buttons formatted to be in a survey layout".

You can replicate their layout structure using radio buttons and the CSS provided.
