Direct Approval Instructions not showing

I have checked Direct Approval for a User Task in my Process Diagram, but the instructions do not appear at the top of the e-mail. What am I missing?

Direct approval by itself will only show up in the user's inbox. The buttons will appear on the main inbox page without having to open or fill out the task.
