forms stuck on end parallel

I have a complex approvals sub-process with 14 parallel approvers, each selected based on criteria in the main process.

Almost all approvals have processed through successfully.  That said, I have two which are stuck and I have not been able to troubleshoot or find anything on answers to help.  In each case, all approvers did their approval and the others bypassed accordingly.  In each case, I have traced each stuck approval through to it's respective "End Parallel Department Approvals" step, accounting for every step for each of the 14 paths.  The only thing that remains is the end of the sub-process to move the document on.  I normally can find and diagnose the issues but am at a loss on this one.

As a note, there were two steps which were not in use (see the voids which are shown in the picture below) which I thought were disconnected before these ran, but I eliminated in trying to troubleshoot.

Going forward, I need to determine two things:

1. What caused this so that I can prevent this from occurring again?

2. How do I move these two documents along to complete the forms process?

In the newly released Forms 10.4.2, we introduced additional features in the Monitor page to skip, interrupt and retry steps. In this case, I'd skip the merging parallel gateway to move the process on to the next step. 

If upgrading to Forms 10.4.2 is not an option, I'd recommend opening a support case so they can provide a database script to more manually update the instance.

This upgrade is awesome, but we are currently on 10.4.0.1256, (and will soon be looking to schedule our upgrade) , so it sounds like I'll be opening a support ticket in the morning  Thanks.

As an update, we just completed our upgrade to 10.4.2 and this feature worked like a charm.  Thank you!
