Creating a Table in the Forms Layout where each Row is to be completed by a different participant and...

I need help on how to Create a Table in the Forms Layout where each Row is to be completed by a different participant (up to 12)  & where each participant is only able to see their own entry and not the others. And where in the Final form tasked to the main approver he/she is able to see all of the entries (combined form).  Is this possible using the Table field?

Each row has 3 columns - Single Line field for Reviewer (their name); Multi line field for Comments; and a Radio Button with 2 choices - approve / reject. (this is not same as Approver).

Is a Table field type the correct way to achieve this or is there another easier way?

When I use one table allowing each participant to add a row, I don't know how to keep each participant from seeing what the others have entered before them.  Do I need to have 12 different tables? If so, how do you combine these or do I just list them in order on final form?

Without having to do a crazy amount of customization, you would want to build out 12 copies of the same form, with 12 tables (labeled Approver 1 through 12).

On each form, delete 11 tables (Except for their Approval) and on final form, you would drag and drop all the sections onto the form and assign to final approver.

 

So Approver 1 would have Approver 1 table and have an approval task with form called "Approval 1" and   Approver 2 would have Approver 2 table and have an approval task and form called "Approval 2 and so forth.

Are the rows prepopulated by a database with preset questions or scenarios for the participants to approve and the person that is supposed to approve them?

The comments are typed in by each participant as well as the selection to approve or reject the form.  They are reviewing a vendor form. Each participant needs to make their comments and approvals without the other participants seeing what they enter. At the end of all the participant comments, a final form goes to the manager to consolidate comments for the vendor. All entries are by participant, no database lookups.

Rose,

Thank you for your response.  That is what I ended up doing, I'm testing it with the group I'm creating this for to get their feedback.  Seems like there should be a quicker way to do this and I was hoping that wasn't the case, but you are right that is the only way as of now.
