Public portal form's lookup to sql server(in the domain) is not working. Is this possible since the web portal is installed on the DMZ?

Yes, it is possible.

The most likely reason for it not to work is if you configure the data source with a domain account, the DMZ server probably can't authenticate with it so the connection would fail.

Try using a SQL user account for the data source to see if that changes anything. If not, then you may not have the right ports open.
