failed to start the workflow

Anyone ever seen a way to block a workflow from starting?

QF says Failed to start the workflow for entry ENTRYNAME.

We have verified that QF can see the workflow service, since we are able to select the workflow to start, then we get this message trying to start it. I can't find anything in the workflow designer that prevents workflows from starting. QF doesn't say any particular reason why it can't start. In my experience they can only fail once they start, not before they start.

Is this a one-time occurrence or does it happen for all documents on this machine?

It happens every time they try to save a document where they have start workflow configured. I ended up thinking about how I have never seen anyone use this feature, then realized why, since usually we would create a condition to start a workflow when an entry is created and I couldn't think of a good reason not to do it this way so I recommended they go with a condition instead.

The user running Quick Fields needs rights to write to the Workflow queues on the Workflow Server. Quick Fields starts workflows by putting them directly into the queue, bypassing all starting rules.

Oh, I thought workflows were started via the API when called from other LF products, IE: /workflow hosted in IIS. I am sure every time this site is offline Forms is unable to start any workflows.

Forms uses the web API because Forms could be outside of the firewall and opening a single hole in the firewall is better. Quick Fields starts them by talking directly with the Workflow Server (over the same channels as the Designer).
