I have a form that involving Child Protective Services that require the worker to answer the same 6 questions on every interaction on the case. It could be as many as 30 or more interactions prior to a case decision being made. The current form has 12 incidences repeating with instructions to add additional sheets if necessary. Any help would be greatly appreciated.
