When you are viewing the form, go to the Themes tab and click on the custom theme. If you only see Default Themes, you haven't saved any custom themes yet. To do that, use the Customize tab to customize a theme to your liking. Once it is ready, click the green "Add to custom themes" button above the form. If you go back to the Themes tab, your custom theme should appear at the top.
