Inaccurate Reporting

I run a daily report. A user reported a task that's not his (highlighted in red):

Turns out, that task was already completed and doesn't exist:

Why's it showing up on my report? Anyone else having issues with inaccuracies?

Has this been on the daily report more than once? Or was it a one time issue? 

I'm wondering if the task was completed, but hadn't been processed by the routing engine yet. The report may have grabbed it in a weird state where it was already completed by the user but still waiting to be processed.

Ironically, he wasn't even the person on the team that processed the task … someone else processed the task.

It's wasn't on the report but then showed up on 7/10 and has been on the report since. I just reran the report and it's no longer there. The only thing I did was go to monitor and review the task to help troubleshoot. Maybe because I opened the process info, it reset itself and is now fine. I'm going to monitor this report more closely now to see if there are other instances that no one is reporting to me. Thanks!

UPDATE:  8/23/2019 - This is still an issue. But I am only seeing this with two processes.
