What's the minimum steps needed to create a form fillable PDF?

I am getting my head around filling out a pdf with a workflow and I am getting " the entry was not a pdf" error. I am missing a simple step. Please shed some light...

Your fillable .pdf needs to be attached to the entry created via the "Attach Electronic Document" activity.  Then, the "PDF" section of the "Fill Out PDF Form" needs to be mapped to "Other Entry" where you pick the activity as "Create Entry" and the entry is "Output Entry"

I usually do this a bit differently.

 

I keep the fillable PDF in Laserfiche in a hidden folder, then use Find Entry using the entry ID. 

 

Copy the entry (the Fillable PDF) to a new folder, then use the Fill PDF Form on the copied entry.
