Workflow that seems to run on folders other than intended one

I have a workflow that populates some template data and moves files to certain folders based on the template information.  I have it set to run on entries in a specific folder (folder A) and then move them to sub folders in that specific folder (folder A).

 

When I run the common searches and see instances   that have an original path that is not the one I specified the workflow to look through.

Can you post your starting rules? Also how you are finding the items? Find Entries? A Search query?

I think I have it working correctly, I have it set to run on a condition about document count and added one more condition to look in the "Entry:Parent ID" of the folder I want.

 

I was using find entries as the search method, it was set to search in the folder I created the condition rule for.

As an FYI
You could have used a starting rule such as this

Any document created in the Record Folder would have kicked off your workflow.
