Only 1 row showing up, despite the fact I picked 5 fixed rows and labeled them

Just started a form, I put in a table and choose 5 fixed rows, but on the form only the first row is showing up. Is there a setting I'm missing?

Never mind, on the design side there is only 1 field, but on the form in preview they are all there.
