I am trying to test one of the Business Process Library's forms (the one called:  Records Disposition Request) and it is getting suspended with these errors:  LFF502-UnexpectedError; and further down:  (404) Not Found.

I know that we are set up correctly as we have been using forms for years without any problems, and I have used forms from the Business Process Library in previous years without this problem.

I have compared the links on this form against the other forms and they are the same, so I can confirm that the server location it is looking for is correct.

A workflow terminates with each try and I think this might be where something isn't quite right:

  However, I have confirmed that the forms server that Workflow is looking for is also the exact same that other Business Process workflows are set up for.

What else could be wrong?
