Prompt message - Why does it pop up?

We get this message every so often when activating a workflow to move a document into another folder. Any idea why. It doesn't come up all the time, just sporadically. 

 

This document has changed on the server and must be reloaded before it can be modified. Do you want to get the latest version now?

This occurs for a couple of reasons.

1.. If there is more than one workflow that hits that document and is acting on the document.

2. Someone else has the document open and makes changes while you have the document open.

There might be more, these are what I have seen
