Creating a fillable pdf but the fields are not in the correct place when using the Fill Out PDF activity

I edited a pdf and made it fillable. The fields are in the right place in Adobe. I added it to a Fill Out PDF activity and this is what it looks like. This has happened for several pdf docs. Any ideas?

Hi Jim,

What's going on if you put values in the field from your Workflow?

How is the document when it is populated?

Thank you for your response!!!! It was simply a display issue. The pdf was populated correctly when data was added and the pdf was saved.
