Workflow Subscriber Trace reports a different starting path

We have a simple workflow rule that's been running great for awhile and stopped today.

One of the starting conditions is that the entry be in a certain path, which it is; however, the subscriber trace claims it's in a different folder (and varies if another person tests it):

(You can see the Live Event viewer is correct)

 

The client shows the correct starting folder:

 

And the starting rule:

 

Any ideas why the subscriber trace is reporting an inaccurate path and why the workflow won't start?

It looks like you accidentally collapsed the list of starting rules evaluated in the Subscriber Trace. Try mousing over between the "Starting rules" tab and the conditions and, when the cursor changes, dragging the conditions pane down. That should let you see all starting rules evaluated on that entry.
