unable to upload document to forms

Our AD service account for laserfiche changed and after the change, laserfiche form stopped prepopulating some fields and won't allow to upload document. Is there specific setting within form which I have to apply changed password for AD service account used by laserfiche?

I had a huge issue when our Active Directory crashed many years ago. I believe we were on version 9.x. I tried uninstalling and reinstalling all of Laserfiche and that didn't fix anything. Where we believe the issue centered was in the SQL database. I had tried backing up the database, re-imaging the SQL server but that didn't help either. When we upgraded to 10.x the issue was resolved, but I always suspected it was in the configuration of SQL database. Hopefully you get some less foreboding suggestions on here :) ! !
