Working on a new form. 

QUESTION 1:  How can I get these checkbox columns to line up better?  Notice that some of them aren't even showing beside the boxes.

QUESTION 2:  Can I change the order so that it flows alphabetically down the first column, then the second column and all the way through?

QUESTION 3:  Can I get it to show only those ones that were picked on the submitted form?  (Making the final record smaller.)
